RewardSurvey (“we” “us” or “our”) is committed to protecting your privacy. We appreciate your trust in us. To get the full benefit of RewardSurvey, we ask you to provide us with certain information about yourself at different points on our sites and application interfaces. We also collect information about you when you visit our site through the use of various technologies as described in more detail below. The purpose of this policy is to inform you of what information we collect, why we collect it, what we do with it, how we protect it and how you can access it or change it.

Please note that third-party operated sites that link to or from our site, services, products and other functionality available on our site but hosted by a third party (i.e., a third party API), are not covered by this policy. We encourage you to read the privacy policies of every site that you visit. For your convenience, this policy is linked to every page where we ask for information on our site. You may remove certain information about yourself from our RewardSurvey database at any time. You also have the opportunity to correct or change the information you have provided to us by emailing instructions to customerservice@RewardSurvey.com. A Special Note About Children

Persons under the age of 16 are not eligible for any offers presented by RewardSurvey. Our site is not intended for or directed to children under the age of 16. If you are aware of an instance in which a child under the age of 16 has provided personal information to us, please let us know (through the below contact information), so that we may promptly delete such information or take other appropriate action.

Submitted Information

We request certain information from you on our registration, survey, and delivery forms. Some of the information we request may be considered sensitive personal data related to household income, personal or business finances, health, family and demographic information. This information is used for customer service, to understand your areas of interest, and guide us in selecting prospects for future surveys. We share this and other personal information, such as name, address, phone number, email address and billing information with the publishers of the magazines you request, and with third party product fulfillment companies. In some instances, we also partner with other companies to provide survey or reward services on our behalf as well as on their behalf, and to host some of their services, such as third-party survey companies. Thus, these third-party companies may also collect your information as a result of this partnership. We will endeavor to notify you wherever we are collecting information that may be shared with these third-party partners, unless otherwise clear to you.

Credit/debit card information

When you place an order, we will ask for your name, mailing address, your phone number and credit/debit card information. This information will be used to process your order and send you the product you requested. We will share credit/debit card information with outside companies that are part of the process of fulfilling your order.

Tracking Technology

If you have been offered our products and services through one of our marketing partners or affiliates (see "Use of Information" section below), such as retailers who offer magazine subscriptions or have our paid listings and surveys on their sites, the information you enter through their site may be seen or inadvertently shared with that partner or affiliate. We may also share some of your information with them to track conversion rates and award points, where applicable. To do so, we may use a technology called clear GIFs, which automatically and securely transfer the information you enter. In the future, we may also use other similar technologies in our offers for the same purpose. If we do so, we will modify our privacy policy accordingly. See the "Notification of Changes" section below.

Log Files

We log IP addresses in order to improve security, analyze trends and administer our offers. We may track your movement within the site, the pages to and from which you are referred, access times and browser type. We (or a third-party platform we work with) also may place cookies (discussed below) on your web browser, and assign a unique identifier (a “UID”) to those cookies. These UIDs may be used, for instance, to customize content or marketing to you.

Cookies

RewardSurvey, like most other Web sites, uses cookies when you visit our site. Cookies are small bits of data that remain on your computer hard drive that allow us to make your visit to our site more enjoyable and provide you with a more personalized experience. Cookies are used for various purposes, such as remembering information that you have already entered, for instance your password and preferences, so that you will not have to re-enter them every time you visit and keeping information previously provided for certain services available on the site. They also tell us general information about how users visit our site, such as which pages they've visited and what services they use, so we can further customize the information we provide. This will make our site more valuable to you, since the information provided will be tailored to your tastes. Please note that despite such setting, our site does not respond to such requests and will continue to use cookies for tracking purposes even if you have set your browser to not allow them. You can also manually delete all cookie files from your computer whenever you choose. We use cookies only for your convenience and to enhance your visit to our site.

Combination of Information

We may combine any of the information that we collect about you, such as your registration information and a unique ID (UID). Sometimes, the third parties we work with will also combine information they collect about you – for instance, by connecting a UID they have used to identify your browser to information about your browser activity on other web pages. To opt out of this information being used for ad targeting purposes, please see the section titled “Interest-Based Advertising and Your Marketing and Advertising Choices” below. We refer to all of the information that we describe and collect in this Privacy Policy as your “information.”

Use of Information

We use the information we collect from you in the following ways:

  • RewardSurvey- We may use your information including email address for selecting future survey panels, our own marketing and other commercial purposes. If you do not want us to share your data with third parties for their own commercial or marketing purposes outside of the services they provide to us, please see below opt out link.
  • Contractual Relationships- We provide your information to, and make it available to, our third party service vendors with whom we have contractual relationships for the purpose of servicing or processing of our sweepstakes, and your rewards and points. These vendors also provide various services on our behalf such as customer service, email marketing, and web hosting. These vendors are not allowed to sell, rent or lease your personally identifiable information and are not permitted to use your information for any secondary purpose.
  • Publishers- When you request a magazine, we necessarily have to forward your contact and address information to the applicable magazine publisher to determine your eligibility for fulfillment. The magazine publishers for the titles you have chosen to receive may use the information you provided to present you with other opportunities in accordance with their own privacy policies. When requesting a magazine, we encourage you to read the privacy statement of the publisher for that magazine; we do not review the privacy policies of the publishers in our program and are not responsible for their actions.
  • Survey Sponsors- Companies and organizations that may host or sponsor RewardSurvey surveys may receive survey response results at the time of collection, and/or UIDs for purposes of tracking conversion and awarding points.
  • Retailers/Special Offers- When you choose to accept a special retail offer or redeem a non-magazine reward, we forward your contact information to the applicable retailer and/or sponsor for fulfillment. The retailers and/or sponsors for the offers you have chosen to receive may use the information you provided to present you with other opportunities in accordance with their own privacy policies.
  • Third Party Marketing Affiliates- If you accept one of our offers by linking through one of our partners or affiliates' operated sites, your information also may be shared directly or indirectly by you, with the partner or affiliate which originated the link to our site. These partners and affiliates may use the information you provided to present you with other opportunities in accordance with their own privacy policies. We are not responsible for the privacy policies of our partners and affiliates.
  • Exceptional Circumstances Please note that if RewardSurvey or any of our lines of business is sold or disposed of as a going concern, whether by merger, sale of assets or otherwise, the relevant customer database of RewardSurvey could, in whole or in part, be sold as part of that transaction. We also may have to release personally identifying information when it is necessary to comply with judicial or other government subpoenas, warrants or orders.

Interest-Based Advertising and Your Marketing and Advertising Choices

a. Interest Based Advertising. We work with third party platforms to help serve advertising, which may be through display, email, direct mail, or other methods. These third-party platforms use technologies like cookies and GIFs that we describe above. These platforms may use these technologies to (i) inform, optimize, and serve ads based on past visits to our sites and other sites (or bases on other information these third parties have collected about you from other sources), (ii) serve our ads on other sites, (iii) report how our advertising performs (for instance, whether advertising impressions may be related to visits to our Site or to other transactions or activities), or (iv) analyze website visits or ad campaigns in other ways. This type of advertising – i.e., using activity collected over time and across sites to serve more relevant ads – is often called “interest-based advertising.”

Similarly, when we work with these ad platforms, we or they may place cookies (or similar technologies) that have been associated with de-identified (such as through “hashing” de-identification formulas) demographic or other data linked to or derived from data you voluntarily have submitted to us (e.g., your email address) and that we may share with a service provider to trigger or suppress an ad. Through these and other identifiers (which are de-identified but may have been derived from your contact information), service providers we work with may target, measure, or analyze advertising to you on a variety of other platforms (for our products or other products), such as through direct mail, email, or “connected” television channels, as well as social media platforms.

b. Your Choices: Opting Out of Interest-Based Advertising. If you are interested in more information about tailored browser advertising and how you can generally control cookies from being put on your computer or mobile browser to deliver interest-based advertising, you may visit the Network Advertising Initiative’s Consumer Opt-Out Link and/or the Digital Advertising Alliance’s Consumer Opt-Out Link and on those sites, you may opt-out of receiving tailored advertising from companies that help serve those ads. Opting out in this way won’t prevent you from receiving ads – it will only (in many cases) make those ads less relevant, because they won’t be tailored to your interests. Also note that the above opt-outs are cookie-based, so if you delete your cookies (or change your browser), you’ll need to opt out again. We do not control any of the above opt-out links and are not responsible for any choices you make using these mechanisms or the continued availability or accuracy of these mechanisms.

Further, we may work with data providers who collect web log data from you (including IP address and information about your browser or operating system), or place or recognize a unique cookie on your browser to enable you to receive interest-based ads or content through web, mobile or other media channels. The cookies may reflect de-identified demographic or other data linked to data you voluntarily have submitted to us, e.g., your email address, that we may share with a data provider solely in hashed, non-human readable form. Subscription offers may contain links to other sites. Please be aware that RewardSurvey is not responsible for the privacy practices of such other sites. We encourage you to read the privacy statements of each web site that collects personally identifiable information. This privacy statement applies solely to information collected by RewardSurvey.

Security

RewardSurvey takes measures to protect the personal information you enter by using Secure Socket Layer (SSL) encryption, which is recognized as an industry standard in encryption technology, whenever you enter personal information.

Special Offers and Choice/Opt-Out

After you complete our subscription forms, your answers and contact information are necessarily sent to the publisher of the magazines you request so they can determine your eligibility and fulfill your subscriptions. These publishers may also contact you with special offers. From time to time, we may present you with specially selected offers from RewardSurvey or one of our partners with whom we contract. Out of respect for your privacy, you can forfeit any of these opportunities by emailing us at customerservice@RewardSurvey.com to remove yourself from receiving future offers.

Site and Service Updates

We communicate with you to provide requested services and on issues relating to your subscriptions. We also send site and subscription service updates. You are not able to unsubscribe from these important announcements, which contain important information about your subscriptions and RewardSurvey.

Correcting/Updating Personal Information

If you want to change any personal information you previously provided to us (such as your address) you can do so by emailing us at: customerservice@RewardSurvey.com.

Notification of Changes

We may revise this Privacy Policy from time to time. The most current version of the policy will govern our use of your information and will be available at https://rewardsurvey.com/Home/PrivacyPolicy. If we make a change to this policy by continuing to access or use this site after those changes become effective, you agree to be bound by the revised Privacy Policy.

As described above, RewardSurvey collects information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular individual or household (personal information). RewardSurvey may use, share, or disclose these categories of personal information for one or more of the indicated business or commercial purposes.

Information drawn from any of the information identified in this subdivision to create a profile about a consumer reflecting the consumer’s preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities, and aptitudes.


In the preceding 12 months, Reward Survey has shared and/or sold, by the way of redirected links, limited consumer personal information in the form of unique identifiers and qualities to third-party partners who conduct their own surveys. RewardSurvey may also use your information in the future to promote other third-party products and services. However, we do not sell the personal information of consumers we actually know are less than 16 years of age without affirmative authorization.

Right to Opt-Out of the Sale of Personal Information

You have the right, at any time, to direct RewardSurvey not to sell your personal information by visiting our online portal Contact Us or calling 877-202-9589. In order to submit your request, you must log in to your account, or if you do not have an account, by matching data points provided by you with data points already maintained by RewardSurvey that we have determined to be reliable, including, but not limited to, your name, phone number, email address, or mailing address. You may use an authorized agent to submit a request to opt-out of the sale of personal information on your behalf if you provide the authorized agent written permission to do so.

RewardSurvey has also disclosed consumer personal information to service providers and other third parties for a business purpose. In particular, RewardSurvey has disclosed the above identified categories of consumer personal information, which have been obtained directly or indirectly from you, third parties such as our marketing affiliates, service providers, data resellers and others, to service providers and magazine publishers. Examples include email, cloud, and e-commerce software providers, merchant processors, fulfillment houses, affiliate marketers who market our products and services on our behalf, award, rebate and point processors with whom we contract, and other third-party service providers.

Right to Know About Personal Information Collected, Disclosed, or Sold

You have the right to request that RewardSurvey disclose to you: (1) the categories of your personal information collected; (2) the categories of sources from which your personal information was collected; (3) the categories of your personal information that were sold or disclosed for a business purpose, if any; (4) the business or commercial purpose for collecting and/or selling your personal information; and (5) the categories of third parties with whom we share your personal information. You also have the right to request to receive the specific pieces of your personal information collected by RewardSurvey to the extent such information is maintained by, and available to, RewardSurvey. .

To submit a request, please go to Contact Us or call 877-202-9589. We will ask you to log in to your account, if you have one, to verify your request; otherwise, you can submit a request as a non-accountholder.

In order to verify you are the person about whom we collected personal information you must verify your identity by matching at least two pieces of Personal Information provided by you during your interactions with us or purchase transaction with Personal Information maintained by either the website operator/retailer or us, that we have determined to be reliable.

However, if your request involves receiving specific pieces of Personal Information (rather than categories of Personal Information), we must first verify your identity by matching at least three pieces of Personal Information provided by you with Personal Information maintained by us. And, if you are not an accountholder, you must provide a signed declaration under penalty of perjury that you are the consumer whose Personal Information is the subject of the request.

We cannot respond to your request to provide any personal information if we cannot verify your identity using the information provided.

Right to Request Deletion of Personal Information

You have the right to request that RewardSurvey delete personal information that we collected from you and retained, subject to certain exceptions.

In order to process your request, you must first verify you are the person about whom we collected personal information. If your request relates to non-sensitive information, we can verify your identity by matching at least two data points provided by you with two data points already maintained by us or a third-party with whom we contract with to provide services to you. Please log in to your account to make the request, so our verification process can be easier.

If your request involves sensitive personal information, we must first verify your identity by matching at least three pieces of Personal Information provided by you with Personal Information maintained by us or a third-party service provider that we have determined to be reliable. And if you are not an accountholder, you must provide a signed declaration under penalty of perjury that you are the consumer whose Personal Information is the subject of the request.

We endeavor to respond to a verifiable consumer request within forty-five (45) days of its receipt. If we require more time (up to 90 days), we will inform you of the reason and extension period in writing. We will deliver our written response by mail or electronically, at your option. The response we provide will also explain the reasons we cannot comply with a request, if applicable.

Right to Non-Discrimination for the Exercise of Privacy Rights

We will not discriminate against you for exercising any of your CCPA rights. This means that, unless permitted by the CCPA, your exercise of these rights will not result in RewardSurvey: (1) denying you goods or services; (2) charging you different prices or rates for goods or services, including through granting discounts or other benefits, or imposing penalties; (3) providing you a different level or quality of goods or services; or (4) suggesting that you may receive a different price or rate for goods or services or a different level or quality of goods or services. However, please note that we cannot process points, orders and deliver services if we have to delete, or we cannot collect, certain of your information.

Designating an Authorized Agent

You may use an authorized agent to submit a request to opt-out of the sale of personal information on your behalf if you provide the authorized agent written permission to do so.

He or she must provide their name, email address, phone number, and written permission from you that includes your contact information. We will contact you using the information provided and verify your identity directly with us. We cannot respond to your request to provide additional disclosures or delete any personal information if we cannot verify your identity and the authorized agent’s authority to make the request on your behalf. Written permission is not required if the agent provides proof of power of attorney pursuant to Probate Code sections 4000 to 4465, or you are submitting the request on behalf of your minor child.

Other California Privacy Rights

California’s “Shine the Light” law (Civil Code Section § 1798.83) permits users of our online services that are California residents to request certain information regarding our disclosure of personal information to third parties for their direct marketing purposes. Effective Date

This privacy notice was last updated on 06/25/2020. RewardSurvey reserves the right to amend this privacy notice at its discretion and at any time. When we make changes to this privacy notice, we will post the updated notice on our website and update the notice’s effective date.
